**Key Responsibilities**
As an AIM practitioner, your primary responsibilities will include:
- Facilitating advanced illness management and palliative care for patients
- Developing and implementing individualized treatment plans in collaboration with patients, families, and healthcare teams
- Providing diagnosis, treatment, and follow-up care for patients referred to our program
- Participating in evening/weekend call as required, conducting on-call services in a clinically competent and responsive manner
