We are seeking an experienced Nurse Practitioner to join our team in providing compassionate, high-quality care to patients with advanced illnesses. As a member of our interdisciplinary team, you will work collaboratively to develop individualized care plans that address the unique needs of each patient.
Job Description:
- Assess patient needs, develop care plans, and implement interventions to improve quality of life.
- Provide education and support to patients and families on disease management, symptom control, and end-of-life care.
- Participate in multidisciplinary rounds and contribute to ongoing care planning and decision-making.
Qualifications:
- Masters degree in Nursing, with specialization in Family or Adult Medicine preferred.
- Current licensure/certification as a registered nurse (RN) or APRN.
- Minimum 2 years' experience in palliative care or a related field.
