You will work closely with the interdisciplinary team to assess patient needs, develop individualized care plans, and provide education to patients and families.
Your Key Responsibilities:
- Conduct thorough assessments of patients' physical, emotional, and social needs.
- Develop and implement comprehensive care plans that address pain management, symptom control, and quality of life enhancement.
- Provide education to patients and families on disease management, self-care techniques, and end-of-life care.

Requirements:
- Registered Nurse with 2 years of prior skilled experience.
