As an Advanced Illness Management (AIM) Palliative RN, you will play a crucial role in providing end-of-life care to patients in their homes, assisted living facilities, or skilled nursing facilities. You will work closely with the interdisciplinary team to assess patient needs, develop individualized care plans, and provide education to patients and families.
Your Key Responsibilities:
- Conduct thorough assessments of patients' physical, emotional, and social needs.
- Develop and implement comprehensive care plans that address pain management, symptom control, and quality of life enhancement.
- Provide education to patients and families on disease management, self-care techniques, and end-of-life care.
- Collaborate with the interdisciplinary team to ensure seamless coordination of care.

Requirements:
- Registered Nurse with 2 years of prior skilled experience.
- Demonstrated ability to provide quality patient care.
