As a Nurse Practitioner - Advanced Illness Management , you'll play a crucial role in delivering personalized care to those with advanced illnesses and providing palliative support. This is your chance to make a monumental difference in the lives of patients and their loved ones.
Key Responsibilities :
- Collaborate with a Professional Services Corporation Physician, primary care physicians, and an interdisciplinary team to deliver focused advanced illness management and palliative care.
- Handle advanced illness management from assessment through discharge, evaluating disease-specific changes, patient function, nutrition, cognition, and the impact of illness on quality of life.
- Develop individualized care plans in consultation with patients, families, and the care team.
- Address identified care needs promptly, supporting the overall wellness of patients.
- Participate in evening and weekend on-call services when necessary.
- Prescribe medications, order tests, and manage treatment plans according to professional standards and state laws.
- Communicate clinical findings with the PSC Physician regularly, adjusting care plans as needed for optimal patient outcomes.
- Foster strong working relationships with supportive care team members to enhance patient care.
- Document all clinical findings and treatment plans according to industry standards.
- Stay updated on Palliative Medicine Quality Measures and report as required.
