As a Nurse Practitioner - Advanced Illness Management , you'll play a crucial role in delivering personalized care to those with advanced illnesses and providing palliative support. This is your chance to make a monumental difference in the lives of patients and their loved ones.

Key Responsibilities :

  • Collaborate with a Professional Services Corporation Physician, primary care physicians, and an interdisciplinary team to deliver focused advanced illness management and palliative care.
  • Handle advanced illness management from assessment through discharge, evaluating disease-specific changes, patient function, nutrition, cognition, and the impact of illness on quality of life.
  • Develop individualized care plans in consultation with patients, families, and the care team.
  • Address identified care needs promptly, supporting the overall wellness of patients.
  • Participate in evening and weekend on-call services when necessary.
  • Prescribe medications, order tests, and manage treatment plans according to professional standards and state laws.
  • Communicate clinical findings with the PSC Physician regularly, adjusting care plans as needed for optimal patient outcomes.
  • Foster strong working relationships with supportive care team members to enhance patient care.
  • Document all clinical findings and treatment plans according to industry standards.
  • Stay updated on Palliative Medicine Quality Measures and report as required.

Qualifications :

  • Master's degree in Nursing from an accredited institution.
  • At least two years of experience as a Nurse Practitioner.
  • Preferred specialties in Family Medicine or Adult Medicine.
  • Experience with value-based care practices is advantageous.
  • Ability to work independently in an advanced practice role.
  • Licenses and Certifications :

  • Active Registered Nurse and Nurse Practitioner licenses as required by state law.
  • Collaborative agreement with a supervising physician.
  • DEA licensing, including federal and state if required.
  • ACHPN certification in Hospice and Palliative Care is preferred.
  • Valid driver's license and current automobile insurance.
