Patient Care Specialist Overview

The Patient Care Specialist will work closely with a multidisciplinary team to deliver high-quality patient care, focusing on chronic disease management, symptom control, and improving patient outcomes.

This role requires strong clinical skills, excellent communication abilities, and a compassionate approach to patient care.

The ideal candidate will have a minimum of two years of experience as a Nurse Practitioner and a Master's degree in Nursing from an accredited school of nursing.

About the Role

  • Collaborate with healthcare professionals to develop individualized treatment plans.
  • Manage chronic diseases, controlling symptoms and improving patient outcomes.
  • Communicate effectively with patients, families, and healthcare professionals.

Key Responsibilities:

  • Assess patients' physical, emotional, and spiritual needs.
  • Develop and implement patient care plans.
  • Provide education and support to patients and their families.

Requirements

  • Current license as a registered nurse or equivalent required.
  • Active and unrestricted Nurse Practitioner certification/license without board actions.
  • Certification in Hospice and Palliative Care (ACHPN) preferred.
