Job Responsibilities
* Develop and implement individualized treatment plans for patients with advanced illnesses
* Conduct thorough patient assessments to identify areas for improvement
* Provide diagnosis, treatment, and follow-up care for patients referred to the company
* Participate in evening/weekend call as required, conducting on-call services in a clinically competent and responsive manner
* Communicate effectively with healthcare teams to ensure seamless care coordination
Requirements
* Current license as a registered nurse or equivalent as required by state law for Nurse Practitioner licensure without board actions
* Current Nurse Practitioner certification/license without board actions
* Collaborative agreement with a supervising physician that is part of the Professional Services Corporation, agreeable to supervise work in advanced illness management and palliative care program, as required by state law

The ideal candidate will have excellent leadership, interpersonal, written and verbal communication skills, as well as independent decision making and self-motivation.
