Job Description
The Nurse Practitioner – Advanced Illness Management AIM) CPOM facilitates advanced illness management and palliative care for our patients, ensuring a comprehensive, individualized treatment plan, while demonstrating excellent clinical skills and ethical, relevant judgment.
About the Role
This is a safety-sensitive position that requires the ability to work autonomously in an Advanced Practice Role. The successful candidate will have a strong background in nursing and experience working in palliative care settings.
You will work closely with patients, families, and caregivers to understand their unique needs and preferences, and provide compassionate and patient-centered care that addresses their physical, emotional, and spiritual well-being.
Responsibilities
- Develop and implement individualized care plans for patients with advanced illnesses.
- Collaborate with healthcare teams to ensure seamless coordination of care and effective pain and symptom management.
- Provide education and support to patients and families on managing chronic conditions and end-of-life care.
