Job Summary:
We are seeking a highly skilled Palliative Care Nurse Practitioner to join our team in Advanced Illness Management. As a key member of our multidisciplinary team, you will be responsible for assessing patient needs, developing individualized treatment plans, and providing expert clinical care to ensure optimal quality of life for our patients.
Responsibilities:
* Conduct thorough assessments of patients with advanced illnesses to identify their unique needs and develop comprehensive treatment plans
* Collaborate with physicians and other healthcare professionals to ensure seamless coordination of care
* Provide expert clinical care, including diagnosis, treatment, consultation, and follow-up, under the direction of our Palliative Medical Director or Primary Care physician
* Work closely with patients, families, and caregivers to ensure that their needs are met and they receive the support they require
* Participate in evening/weekend call as required, conducting on-call services in a clinically competent and responsive manner
Requirements:
* Master's degree in Nursing from an accredited school of nursing
* Certification in Hospice and Palliative Care (ACHPN) preferred
* Experience in palliative care and hospice management
* Strong clinical skills and excellent communication and interpersonal skills
* Ability to work efficiently with electronic medical record software
* Knowledgeable of Medicare/Medicaid regulations and principles of Palliative Medicine
