About the Role:

• Conduct thorough patient assessments, establish individualized treatment plans, and communicate effectively with care teams.
• Provide high-touch care, addressing symptoms and stress associated with illness, and prioritize patient needs.
• Work autonomously in an Advanced Practice Role, making independent decisions and taking ownership of patient care.

Requirements:

• Masters of Science in Nursing from an accredited school of nursing or equivalent experience.
• Minimum of two years of Nurse Practitioner experience, preferably in Family or Adult Medicine.
• Certification in Hospice and Palliative Care (ACHPN) is preferred.
