Nurse Practitioner - Advanced Illness Management
As a Nurse Practitioner specializing in Advanced Illness Management, we collaborate with physicians and multidisciplinary teams to deliver personalized palliative care.
Role Responsibilities:
- Conducting assessments, developing treatment plans, and ensuring comprehensive care for patients with advanced illnesses.
- Enhancing the quality of life of our patients through a blend of clinical expertise and compassionate support.
- Providing relief from distressing symptoms while guiding patients and their families through their healthcare journey.

Requirements:
- A minimum of Masters of Science in Nursing from an accredited school of nursing, or an equivalent as allowed under state law.
- Nurse Practitioner specialty in Family or Adult Medicine preferred.
- Value Based Care practice experience preferred.
- Practice experience with Part B billing.
- A collaborative agreement with a supervising physician, that is part of the Professional Services Corporation, that is agreeable to supervise work in advanced illness management and palliative care program, as required by state law.
