Illumination Foundation (IF) is a growing non-profit organization dedicated towards disrupting the cycle of homelessness by providing targeted, interdisciplinary services in our recuperative care centers, emergency shelters, housing services and children's and family programs. IF currently has 13 facilities with 22 micro-communities scattered across Orange County, Los Angeles County and the Inland Empire.Job DescriptionThe Lead Case Manager is responsible for assessing the strengths and needs of homeless clients, developing care plans for clients within their service area, and overseeing client care within their assigned program. The Lead Case Manager will work closely with the Program Manager to ensure contract compliance and outcomes. The pay range for this position is $26.00 - $28.00 per hour, depending on experience. This role will be for our Broadway Recuperative Care and our Buena Park Short term post hospitalization location. ResponsibilitiesClient Care OversightDevelop care plans for clients within assigned programs.Oversee the care plans in partnership with the Program Manager by conducting monthly meetings with care teamsOversee client assessments to evaluate client's need, develop treatment plan for client, monitor progress, provide assistance and referrals, set-up and follow-up with weekly goals for case managers to work on with clients.Provide crisis intervention for all clients within program areDirect Client CareMeet with clients as often as needed to carry out the goals of the care planAssist clients in connecting to any basic needs resourcesSupervisionSupervise assigned cms weekly to discuss client care and assist in strategic problem solving to overcome barriersConduct monthly care team meetings with all relevant members of the client's care teamProvide direct case management to participants as needed; supervise all case management activities carried out by employees, volunteers, and interns to ensure participants receive excellent service.Administrative / Record KeepingTrack client and program data using Google docs and HMISCreate, maintain, and organize client case files with necessary documentationComplete Client Outcome Matrix when due and input into HMISComplete weekly and monthly reportingUse outcome measurementsMaintain updated and accurate case notesComplete BPS forms, weekly goals, and exit formsCreate, fax, email and send documents on client's behalfAttend weekly individual supervision session with immediate supervisor to discuss program organization, client's progress, new strategies to meet client's level of need and improve treatment planCollaborationsParticipate in Case Management Team Meetings and staff meetingsNetwork with community providersMission SupportDemonstrate a commitment to Illumination Foundation's mission and core values, while developing and maintaining respect and harmony with all employees and management.Must demonstrate the ability to grasp new skills / procedures and foster a positive attitude toward changes.Must be an asset to the organization and a positive influence on co-workers, while displaying respect toward clients and others in the workplaceActs in an ethical manner reflecting core values of integrity, transparency, accountability, respect and responsibility.
