JOB SUMMARY:

Essential Job Functions
- Maintains and actively promotes effective communication with all individuals.
- Maintains a positive image of the entity in the community keeping in alignment with our mission, vision, and values.
- Responsible for on-call services, as assigned.
- Assist new residents and families with the adjustment to community living and the schedules and routines of nursing home life.
- Oversees Resident Care Planning as facilitated by the Life Coach Team.
- Coordinates initial staffing/discharge planning with resident, family, and interdisciplinary team.
- Completes Minimum Data Set and Resident Care Plan related to Social Service.
- Recommends referrals to appropriate agencies in cases of financial or psychosocial concerns, or desired return to the community.
- Acts as discharge coordinator. Provides assistance to other staff, the resident and his/her support system at the time of admission and prior to discharge as under HFS132.
- Assists the resident in meeting personal needs in the absence of the responsible party.
- Maintains current social work progress notes and documents interactions as needed.
- Completes or arranges assistance for residents in completing necessary forms including advance directives.
- Compiles information about community health and social services available, and keeps a current resource file on these services.
- Manages Quality Assurance responsibilities.
- Facilitates an environment that encourages residents and families to share any concerns or questions.
- Is the Grievance Officer and coordinates and manages the process.
- Acts as supervisor to Life Coaches and completes performance reviews and supervision.
- Performs other duties, as assigned.
QUALIFICATIONS:
- Commitment to quality outcomes and services for all individuals.
- Ability to relate well to all individuals.
- Ability to maintain and protect the confidentiality of information.
- Ability to exercise independent judgment and make sound decisions.
- Ability to adapt to change.
- Minnesota Social Work License with 4-5 years of related work experience.
- Proven success in working cooperatively with others and relate well to the patients, families, staff, churches and the community in general.
