Hospice Nurse Case Coordinator

Hospice Nurse Responsibilities:
- Supervising and coordinating the care of patients nearing the end of their lives.
- Managing patient visit schedules based on individual needs, collaborating with physicians, and overseeing medication and orders management.
- Providing supervision to LPN's / LVN's / Hospice Aides as per protocol.
- Working collaboratively with a team of medical professionals to deliver holistic palliative care that encompasses physical, emotional, and spiritual support.
Qualifications:
- Preferably an Associates degree in nursing.
- Must possess a current Registered Nurse license.
- Minimum one year of experience in a similar role.
- Prior experience in a hospice setting.
- Willingness to be on-call as required.
- Must have reliable transportation and a valid driver's license.
