Role and Responsibilities -
The Regional Director is responsible for overseeing and evaluating patient services at the regional level within the home care setting. They will collaborate with clinical management to ensure high quality outcomes, patient and staff satisfaction, and revenue goals achievement. The Regional Director may also serve as the Director of Patient Services.
Essential Functions
- Contribute to the development, implementation, evaluation, review, and revision of policies and procedures according to professional standards.
- Establish goals, objectives, and care standards relevant to patient services and personnel functions.
- Monitor treatment plans, facilitate case conferences, conduct patient evaluations, and determine staffing needs.
- Coordinate clinical record reviews on a quarterly basis.
- Maintain effective communication and coordination with physicians, payers, patients/families, and other stakeholders.
- Ensure patient referral, admission, and discharge decisions align with assessed needs and safety.
- Conduct comprehensive interdisciplinary assessments that address medical, social, mental health, and environmental aspects.
- Provide adequate supervision for personnel delivering patient care, ensuring compliance with care plans and documentation.
- Schedule personnel to guarantee appropriate coverage based on authorized staffing levels.
- Recruit qualified personnel and implement strategies for staff retention.
- Review staff performance on an annual basis, with more frequent feedback for low performers.
- Participate in information management and performance improvement programs.
- Collaborate with the Professional Advisory Council on relevant policy reviews.
- Engage in local organizations and activities related to the health professions and community health services.
- Oversee agency performance in areas such as finance, quality, and regulatory compliance.
- RN with BSN credentials required.
- Registered professional nurse, physical or occupational therapist, or medical social worker with a bachelor's degree (master's preferred) and at least two years of experience in home health care and clinical management.
- Must hold a current, valid NYS Licensure in the respective profession.
- Minimum of two years' experience in home health care.
- Previous experience with Medicare home health and management.
- Strong organizational skills, attention to detail, and effective oral and written communication abilities.
- Demonstrated problem-solving skills in the delivery of home health care.
- Ability to prioritize and manage multiple critical tasks simultaneously.
- Adaptable to change and interruptions in the work environment.
- Effective management skills.
- Must possess a valid NYS driver's license and automobile liability insurance, as well as reliable transportation for all weather conditions.
