Job Overview
We are seeking a dedicated and compassionate Case Manager to join our team. Duties
- Conduct thorough assessments of clients' needs, strengths, and challenges to develop personalized care plans and determine eligibility for services.
- Provide crisis intervention and management to clients experiencing immediate distress or emergencies.
- Collaborate with community resources and service providers to coordinate care and support services for clients.
- Monitor client progress and adjust care plans as necessary to ensure effective service delivery.
- Advocate for clients' rights and access to services
- Facilitate mentoring programs and workforce development initiatives to empower clients toward self-sufficiency.
- Maintain accurate documentation of client interactions, progress notes, and service outcomes in compliance with organizational policies.
- Stay informed about relevant laws, regulations, and best practices in social work and child welfare.
Experience
- A Bachelor's degree in social work or a related field.
- Experience in crisis intervention, behavioral health, child welfare, or similar areas is highly desirable.
- Familiarity with Medicaid processes and regulations is a plus.
- Strong mentoring skills with experience in workforce development initiatives is beneficial.
- Proven ability to manage crises effectively while maintaining a calm demeanor under pressure.
- Excellent communication skills, both verbal and written, are essential for successful client interactions.
