Job Title: LTSS Service Care Manager
Duration: 3 months Contract (Potential to extend)
Location: Remote (source from Corpus Christi.)
Shift Type: Mon-Fri 8am-5pm
Pay rate : $25/hr
Day to day responsibilities of this the role:
- SC will have 3 days of visits and 2 days of documentation weekly
- SC will have a caseload and work assigned tasks
- SC will complete H2060 assessment
- SC will fill put required forms for each assessment
- SC will make follow up calls
Describe the performance expectations/metrics for this individual and their team:
- SC will complete documentation within 3-day turnaround time
- SC will stay organized and ensure work they do not fall behind
- SC will maintain a 92% or high in Audit scores
Required Skills/Experience:
- 2 years of experience in a managed care environment or working with people with disabilities and vulnerable populations who have chronic or complex conditions.
Education Requirement:
- Bachelor’s degree in social work, Health Services, Behavioral Science or related field, or equivalent experience. LVN all accepted
Required Certifications:
- Driver’s license is required
