Position Overview:
We are seeking a qualified Care Manager to join the Sunshine State Health Plan team, focusing on pediatric care management. This role involves working closely with members, providers, and internal teams to address healthcare needs and coordinate comprehensive care. The position is remote, but candidates must reside within Hendry or Glades counties, or specifically in Immokalee (Collier County).
Key Responsibilities:
- Proactively outreach to members on a scheduled basis to assess unresolved healthcare needs and address barriers to care.
- Collaborate with providers, suppliers, vendors, and internal departments to coordinate care.
- Conduct monthly or quarterly phone contacts with members, annual needs assessments, and quarterly in-person visits as required.
- Accurately document all member interactions, outreach efforts, and care coordination activities.
- Ensure compliance with contract requirements set forth by AHCA and DOH, monitored through chart audits, caseload reviews, and recorded call evaluations.
- Apply trauma-informed practices when working with members, especially those involved in the child welfare system or community mental health settings.
Qualifications:
Required:
- Education:
- Master’s degree in behavioral health or social work, or a degree from an accredited School of Nursing.
- Relevant certifications: Licensed Mental Health Counselor (LMHC) or Licensed Clinical Social Worker (LCSW).
- Experience:
- Minimum of 2 years working with the pediatric population.
- Proficiency in Microsoft Office (Outlook, Word, Excel).
- Other Skills:
- Strong understanding of healthcare coordination processes and documentation.
Preferred:
- Experience in:
- Child welfare systems.
- Community mental health agencies.
- Trauma-informed care practices.
- Bilingual skills.
Work Schedule:
- Monday to Friday, 8:00 AM to 5:00 PM (EST).
Training:
- Initial training includes 4 weeks of virtual instructor-led sessions, following a brief orientation with the supervisor during the first two days.
Additional Details:
- Duration: 6 months, with potential for extension or transition to full-time employment.
- Target Start Date: February 10, 2025 (Clear by Date: January 27, 2025).
