Job Title: Care Manager (RN)
Duration: 8 months Contract (Potential to Extend)
Desired Start Date/End Date: 3/24/2025 - 11/28/2025
Location: West Central (Dayton, OH) - Remote
Desired Start Date / End Date : 3/10/2025 - 8/8/2025
Shift Type: 8am-5pm EST , Mon-Fri
Pay rate: $40 to $45 per hour
Job Duties:
- Required to do field visits (member-facing)
- Two different types of caseloads (one working with over 60 waiver population; other will be working with the under 60 population)
- Assessments face-to-face visits for maintaining safety welfare; audit their caseloads
Required Skills:
· Must have Home Health background.
· Documentation requirements: run weekly reports to ensure visits are completed and documentation is completed timely.
· Behavioral Health and experience with working with mental health and substance abuse .
Education:
- Associate’s degree of Nursing
Licensure:
- Registered Nurse (OH or compact)
Salary : $40 - $45
