Responsibilities
What You Will Do:
- Organize, develop, manage, and direct the home health functions of your community health hub
- Manage a team of clinicians to ensure our patients receive exceptional care in their homes
- Assess, plan, direct, and evaluate each patient’s care plan to ensure it meets the patient’s needs
- Conduct patient visits (as needed)
- Manage emergency situations in a prompt, precise, and professional manner
- Complete medication administration and treatment needs for all assigned patients
- Provide nursing care in accordance with applicable regulations and Company policy
- Prepare and maintain accurate and complete clinical and progress notes on a timely basis
Qualifications
Qualifications:
- Active RN state license
- Two or more years of nursing experience (supervisory experience preferred)
- Two or more years of home health nursing experience preferred
- Valid drivers’ license
