- On call schedule: 7 on, 7 off
- Work collaboratively with physicians, nurse practitioners, nurses, therapists, social workers, and caregivers to deliver comprehensive and coordinated care to our patients in their homes – conduct patient home visits
- Assess, plan, direct, and evaluate each patient’s care plan to ensure it meets the patient’s hospice needs
- Manage emergency situations in a prompt, precise, and professional manner
- Counsel patient and patient’s family on end-of-life needs
- Complete medication administration and treatment needs for all assigned hospice patients
- Provide nursing care in accordance with applicable regulations and Company policy
- Prepare and maintain accurate and complete clinical and progress notes on a timely basis
- Active RN state license
- Valid drivers’ license
- Two or more years of applicable nursing experience
- Eligibility for certain benefits may depend on employment status
