IMPaCT is looking for people with this natural gift of caring to employ them as full-time Community Health Workers." At IMPaCT Care, we specialize in partnering with healthcare organizations to recruit and hire community healthcare workers. We are currently hiring for "Senior Solutions" to fill this exciting new position. The main requirement: Are you helpful? If yes, keep reading.

Community Health Workers (CHWs) are a workforce to enhance the services that Senior Solutions delivers to our clients. CHWs come from the communities they serve and are trustworthy individuals who provide social support, navigation, care coordination, health coordination, and advocacy for their community. CHWs are a boots-on-the-ground workforce who can cost-effectively address the upstream causes of poor health - such as discrimination, loneliness or food insecurity - before they turn into full-blown health crises. Senior Solutions Community Health Workers will have the power to transform communities by building relationships and connection, provisioning holistic, creative, and hands-on support, and walking alongside people to help them address the barriers to good health.

Community Health Worker Responsibilities:

  • Meet with assigned clients in clinic, hospital, or home
  • Build and maintain professional and supportive relationships with assigned clients
  • Get to know assigned clients as people an help them do the thing that they will improve their health such as offering a listening ear, connecting them to resources like food delivery, or working out with them at the YMCA
  • In-person client working including required home visits and completion of assessments
  • Attend meeting with CHW Supervisors and provide detailed updates on client cases
  • Serve as a link between the community and health/social services to expand access to care and improve health outcomes
  • Facilitate access to social services and social support
  • Improve quality and cultural competence of Senior Solutions service delivery
  • Motivate, teach, and help clients build wellness and other skills
  • Adheres to all company policies, including dress code with proper PPE, time off, call outs, etc.
  • Clocks in and out of every shift they work on the appropriate systems associated with the client or clients they are servicing.
  • Observes clients to recognize changes in their behavior or health, then notifies the appropriate personnel of Senior Solutions
  • Assist with additional administrative responsibilities including: admissions paperwork for new clients, training new Caregivers, installation of PERS, meal delivery, field recruiting, executing hiring days, and other administrative tasks as

needed and as assigned

  • Other duties as assigned

Role Requirements:

  • High School Diploma, GED, or other equivalency (preferred)
  • 1 - 2 years of experience in a customer-facing role of any type
  • Knowledge of the communities in which you will be working
  • Successful completion of all initial and ongoing training including compliance training, Level 2 and 3 training, Lead Caregiver training, and CHW certification.
  • Ability to communicate effectively in both written and verbal forms
  • Strong interpersonal, problem solving, advocacy, and evaluation skills
  • Ability to bend, climb, stoop, and stand an average of 5 hours per day
  • Ability to lift 30 pounds
  • Possesses a valid driver’s license and car insurance (required)
  • Possesses a valid second form of ID, such as a social security card
  • Must have reliable transportation with the ability to cover your branch office county.
  • Willing to maintain skills through continued education
  • Must be able to pass a background check and drug testing per company standards
  • Must have a smartphone
  • Willing to maintain skills through continued education and required training for this position

Work Environment:

  • Pick up of supplies will be conducted at a designated drop off point
  • Pick up and delivery of supplies will be conducted in an assigned company vehicle
  • Supplies will be delivered to the homes of SSHC clients and/or an SSHC regional office.
  • Frequent local and regional travel is required for this position Compensation
  • $1,000 bonus for completing CHW certification and actively working in
  • Company vehicle will be made available for travel as appropriate / available
  • Mileage reimbursement eligible for use of personal car
  • CHW Lead caregivers are guaranteed to be scheduled a minimum of 30 hours a week. Tasks will be assigned based on the needs of your assigned area.

Eligibility for CHW Lead Caregiver pay requires the employee to work a minimum of 30 hours per week, maintain a clean driving record, complete all required training, and remain in good performance standards.

CHW Lead Caregivers will receive a total of $1,000 for completing the Community Health Worker (CHW) certification training. CHW Lead Caregivers will receive the certification training payments in 3 increments. Payments will be received on the next pay period after successfully completing the following milestones:

  • $500 will be paid following the successful completion of the CHW training program, completing 30 days of work in the CHW role, and receiving CHW certification from IMPaCT.
  • $250 will be paid following the successful completion of 3 months in the CHW role as long as the employee is still in the CHW Lead position, actively working, and in good performance standards.
  • $250 will be paid following the successful completion of 6 months in the CHW role as long as the employee is still in the CHW Lead position, actively working, and in good performance standards.

Job Type: Full-time

Pay: $15.00 per hour

Schedule:

  • Monday to Friday
  • Rotating weekends

Education:

  • High school or equivalent (Preferred)

License/Certification:

  • Driver's License and Car Insurance (Required)

Ability to Relocate:

  • Knoxville, TN 37940: Relocate before starting work (Required)
