Job Description:
The Community Connections Team (CCT) at Ascension St Thomas Medical Partners is undertaking the mission to address Social Determinants of Health (SDOH). Composed of Community Health Workers and key clinical staff, this team will support 19 outpatient locations in helping patients access available community resources and services.
Key responsibilities for the CHW (Patient Navigator) are assisting clients with accessing available community and medical resources. Will include outreach to patients, assessing barriers to health and well-being, and developing individualized approaches for their needs.
- Identify potential clients, complete initial needs assessment, and maintain follow-up to ensure appropriate guidance to health services.
- Provide informal counseling, education, and social support to clients.
- Assist clients with utilizing available resources, which may include scheduling appointments and assisting with completion of program applications.
- Develop and maintain strong working relationships with external contacts (referrers and community agencies) and internal contacts (departmental team members).
- Maintain accurate client records by documenting all contacts, services provided, and outcomes in order to track all clients and services for future reference.
Desired Qualifications & Skills:
- Knowledge of relevant health and social needs, along with local resources and services, for population in assigned region.
- Core attributes: empathy, natural helpers, listening, resident of and/or in-depth knowledge of the communities served, aligned w/ Ascension's mission and values.
- For Davidson County only: preferred bilingual, fluent in Spanish or Arabic.
Strongly preferred: One or more years of professional experience in areas such as community health, SDOH, and/or social services.

Working Hours: Full-time Monday through Friday, work hours between 8am and 5pm.
Location(s) of Work: Hybrid/Remote -- approximately 50% work from home, and 50% onsite rotating between assigned local clinics.
Job Type: Full-time
Pay: $18.00 - $22.00 per hour
