BILINGUAL COMMUNITY HEALTH WORKER - PART TIME POSITION
Impact Counseling Services, INC. believes that each person deserves the chance to lead a healthy and fulfilling life. It is in this interest that ICS is seeking one full-time and one part-time Community Health Worker (CHW) to implement the Pathways Community HUB Institute Model (PCHI® Model) in East Williamson County. A CHW is an individual who is dedicated to improving individual and community health by acting as a liaison between pregnant and new moms and the medical and social service community. A CHW is a trusted individual within a target population who is able to forge connections with the participants they serve using their knowledge of the community and culture to develop meaningful relationships.
Duties include: home visits, community advocacy between participants and providers, care coordination, case management. Documentation requirements are very reasonable.
Compensation: $23 per hour.

Position Requirements:
- Ability to travel within Williamson County and provide regular in-home services to the cities of Taylor,
Hutto, Granger Bartlet and surrounding areas.
- Education: Minimum of a High School Diploma or GED Equivalent.
- Experience: Prefer minimum of 1 year related experience working with marginalized populations; General
knowledge of Medical Terminology; Knowledge of helpful parenting strategies and how to communicate
these effectively; Knowledge of effective advocacy and consultation.
- Key attributes: Strong interpersonal and communication skills; Ability to communicate with professional
team members and partners effectively, function independently, manage time efficiently, organize workflow
and maintain confidentiality.
- Ability to obtain a CHW certification within one year of employment.
- Ability to work at least one evening a week if client caseload warrants.
- Bilingual in Spanish required
Supervision and Evaluation:
- CHW’s will be supervised and evaluated by the Executive Director. The CHW is required to regularly
attend scheduled large group, small group, and individual supervision.

Major Performance Responsibilities:
- Intake and Screening: The CHW will determine participant eligibility using the PCHI® Model
Demographic and Intake Forms.
- Initial planning and preparation for services: The CHW will develop a plan of care using the 21
Pathways of the PCHI® Model and Intake Forms within the timeframe of the established policy.
- Direct Service: A CHW works with enrolled participants in their homes to identify needs and connect them
to community-based resources and services to remove barriers to care using the PCHI® Model. The CHW
will ensure participants schedule and attend medical appointments with any providers as needed. The CHW
will educate enrolled participants on topics relevant to their situation using the PCHI® Model in a
culturally and linguistically appropriate manner. .
- Care Coordination: The CHW will maintain positive relationships and engage with community service
providers in order to act as a liaison between participants and agencies. The CHW will identify appropriate
federal, state, and local private or nonprofit health and human services programs and provide referrals and
follow-up care for particpants.
- Documentation: The CHW will maintain a full-time caseload of approximately 20 to 30 participants
depending on the level of need assessed by the PCHI® HUB supervisor. The CHW will be
comfortable
utilizing software systems to track and monitor participant progress.
- Outreach and Professionalism: The CHW will attend trainings and monthly meetings for professional
development as required. The CHW will conduct outreach to clinics, community centers, emergency departments and other community sites, to find individuals from the target population eligible for enrollment into the East Williamson County PCHI®.
