Description :
Impact Health is a nurse practitioner group serving skilled nursing facilities across the state with both medical and psychiatric care. Impact Mental Health has per diem openings for a Connecticut licensed psychologists to provide psychiatric consultation evaluations supported by our team of APRNs, LCSWs, and MDs. While providing a strong team atmosphere, Impact allows its providers to practice with autonomy and independence. Providers are allowed to create a schedule that works best for them and their assigned buildings.
Responsibilities :
- Provide person centered care using best practices
- Manage caseload and fulfill psychiatric consultation needs of skilled nursing facility
- Work within a team of social workers, therapists, nurses, and physicians to coordinate care within Impact and the skilled nursing facility
- Provide collaboration with medical team as needed
- Submit required documentation within 24 hours of encounter
- Adhere to state regulations within SNF setting
- Provide assessment, diagnostic, and treatment services
- Provide psychoeducation to patients
- Utilize evidence-based practices within the therapeutic setting
Experience :
Experience in skilled nursing facilities, hospitals, residential care, geriatric care, and psychiatric outpatient preferred
Qualifications :
Active clinical license in psychology in Connecticut
Strong skill set with psychological and cognitive testing
Experiences working with adults and older adults
Strong communication, assessment, diagnostic, and treatment skills
