Responsibilities:
- Conduct thorough assessments of patient wounds, which may be chronic or acute, and develop individualized treatment plans.
- Implement advanced wound care techniques, including debridement, dressing selection, skin grafts, and infection control measures.
- Work closely with other healthcare professionals at each facility to coordinate care and provide updates on patient progress to ensure continuity of care.
- Adhere to state regulations within SNF setting.
- Manage a travel schedule to visit multiple skilled nursing facilities on a consistent basis.
- Family, Geriatric, or Wound Care Nurse Practitioner with active license in Maine
- 1 years clinical experience managing wound care
- Certification in wound care (e.g., CWOCN, CWS, WCC, WOCN) preferred
- Experience in Skilled Nursing Facilities and/or geriatric population preferred
- Strong communication, assessment, diagnostic, and treatment skills
Salary : $110,000 - $200,000
