- Key Responsibilities:**
- **Drive Quality Patient Care and Experience:**
- Ensure 100% of the field team is knowledgeable about and adheres to the iOS process.
- Review and authorize or deny schedule requests outside of Care Insight recommendations based on patient needs.
- Collaborate with the Senior Clinical Director to achieve agency targets, including reducing past-due cases and achieving timely billing and positive patient feedback.
- **Support and Manage Field Team:**
- Educate team members on coverage areas, scheduling, productivity expectations, and charting policies.
- Conduct ride-alongs at least once a year and ensure team members complete semi-annual self-reviews.
- Maintain a consistent presence in communication platforms, responding promptly to team members.
- Address and promptly resolve any inquiries, complaints, or requests from patients and families.
- Coordinate orientation and training for new field team members.
- Lead case conferences and deliver focused educational topics as directed.
- **Implement Clinical Plans and Strategies:**
- Ensure clinical operations align with agency policies, procedures, and recognized best practices.
- Facilitate effective integration with the Impact Service Center, including iOS.
- Maintain a clear and functional staffing plan to meet patient needs.
- Support Quality Assurance and Performance Improvement (QAPI) initiatives.
- **Participate in Clinical Leadership Team:**
- Collaborate with the Senior Clinical Director and Senior Executive Director to meet the needs of patients and the field team, fostering a unified environment.
- Work closely with Schedulers and Office Administrators to streamline operations.
- Encourage a 70% or greater participation rate in the tri-annual team member experience survey.
- Ensure that the agency consistently meets team member experience score commitments.
- Recognize and celebrate team members' achievements, including active involvement in Ripple Awards.
- Qualifications:**
- Proven experience in a clinical leadership role within home health care or a related field.
- Strong understanding of home health care regulations, policies, and best practices.
- Excellent communication and interpersonal skills, fostering a collaborative team environment.
