Overview:
As the Case Worker, you become crucial in the lives of youth aged 17-21 as they navigate the intricate journey of transitioning out of foster care. In this influential position, you will be entrusted with the responsibility of ensuring the delivery of high-quality services to our youth, contributing to the overall success of the program, and upholding the utmost standards of case management.
Responsibilities:
- Provide comprehensive case management responsibilities, ensuring the provision of tailored support and guidance to individuals transitioning to independent living. This includes documenting all of the following information: coordinating resources, conducting assessments, developing individualized plans, and collaborating with relevant stakeholders.
- Supervise youth age 17-21, provide guidance and support to help them achieve their goals as identified on their service plan.
- Conduct thorough interviews and assessments with program participants and their identified family to gather essential information, understand their needs, and create individualized plans for their successful admission into the program.
- Actively fosters a collaborative and supportive atmosphere, contributing positively to team dynamics and ensuring seamless cooperation among colleagues.
- Takes initiative in creating and maintaining a positive work environment, promoting open communication, mutual respect, and a shared commitment to collective success.
- Skillfully prepare teens for both program admission and discharge, providing necessary information, support, and resources to ensure a smooth transition into and out of the program.
- Collaborate and establish partnerships with essential local entities, such as DSS, City Schools, DJJ, and medical and mental health providers, to enhance program effectiveness and support a comprehensive approach.
- Actively engage in and contribute to various tasks and responsibilities as assigned, demonstrating flexibility and a proactive approach to support the overall goals and needs of the team or organization.
Education and Experience:
- A Master's degree in social work from a college or university accredited by the Council on Social Work Education or a field related to social work such as sociology, psychology, special education, or counseling, with a student placement in casework services to children and families or one year of experience in providing casework services to children and families; or
- A baccalaureate degree in social work or a field related to social work including, but not limited to, sociology, psychology, special education, or counseling and one year of experience in providing casework services to children and families; or
- A baccalaureate degree in any field plus two years experience in providing casework services to children and families. (required)

Knowledge, Skills, and Abilities:
- Possesses a profound understanding of trauma, coupled with a high level of proficiency in applying trauma-aware practices with sensitivity and effectiveness.
- Exhibits a high level of proficiency in engaging and collaborating with youth facing social or emotional challenges.
- Exhibits a thorough understanding of the child welfare system, encompassing knowledge of Child Protective Services (CPS) and foster care.
- Possesses expertise in regulations related to licensed child placing agencies, with a specific focus on Independent Living and Treatment Foster Care.
- Demonstrates advanced proficiency in fostering positive and productive relationships, showcasing a deep understanding of effective interpersonal dynamics.
- Exhibits outstanding writing skills, articulating ideas with clarity, precision, and a keen awareness of audience engagement.
Licensed/Certification
- Driver's License required
