Physical Therapist PT

This role is essential in fostering a positive rehabilitative environment for patients recovering from illness, injury, or surgeries. As a Physical Therapist in home health, you will evaluate each patient's physical condition, develop individualized treatment plans, and implement therapy interventions aimed at maximizing each patient’s independence and quality of life. You will engage closely with patients, families, and caregivers to educate them about their conditions and the necessary steps for recovery. • Evaluate patients through physical assessments and medical history
• Develop personalized treatment plans based on individual needs and goals
• Implement therapeutic exercises and training programs
• Educate patients and their families on injury prevention and wellness
• Monitor and document patients' progress toward rehabilitation goals
• Collaborate with other healthcare providers to ensure holistic patient care
• Maintain accurate patient records and documentation in compliance with regulations

• Graduated from an accredited Physical Therapy Program.

• Minimum nine months experience as a physical therapist

• Candidates must possess strong caring interpersonal skills
