This job acts as a leader in the provision of patient care using the nursing process within the framework of the Nurse Practice Act, ANA Code for Nurses, and Scope & Standards of Practice. Effectively delegates, directs, and assists licensed and ancillary team members; assumes accountability for quality patient outcomes; exhibits sensitivity to cultural, ethnic, and religious diversity in all interactions; maintains involvement in activities aimed toward the achievement of unit and the department’s strategic goals and objectives and demonstrates professional responsibility and accountability for his/her own practice and supports the company's philosophy of nursing.

Education
Required - Registered nurse diploma.
Work Experience
Preferred - associate or bachelor’s degree in nursing.
Required – Minimum of 2 Years Related Experience - MUST HAVE ACUTE CARE EXPERIENCE
Certifications
Required - Current RN License in the state of practice
Current Basic Life Support (BLS) certification from the American Heart Association Preferred –
Certification in Case Management (CCM) or Health Care Management (CPHM)
Knowledge Skills And Abilities (KSAs)
Proficiency in using computers, software, and web-based applications.
Effective verbal and written communication skills and ability to present information clearly and professionally to varying levels of individuals throughout the patient care process.

Job Duties
Manages various aspects of the planning process in anticipation of the patient’s movement through the healthcare system.
Uses care management policies and procedures, including applying evidence-based criteria and levels of care and continuing appropriateness.
Manages the care of assigned patients through the healthcare system based on the patient’s individual needs.
Prepares, executes, and reinforces post-discharge care plan.
Adapts behavior to the specific patient population, including but not limited to respect for privacy, method of introduction to the patient, adapting explanation of services or procedures to be performed, requesting permissions and communication style.
Performs Other Related Duties As Required.
