JOB SUMMARY :
This position is a Certified Nursing Assistant who is specifically educated and trained to provide supportive services to residents related to Restorative Nursing Care.
Provides supportive care and coach’s staff as they perform their daily routines, including assistance with dressing, grooming, bathing, walking, eating and other functions of living for restorative care.
Conducts activities and shares common functions with other members of the health care team on the unit. Assists in providing a positive physical, social, and psychological environment.
Follows all Villa’s policies and procedures.
DUTIES / RESPONSIBILITIES :
- Complete accurate and timely clinical record documentation per facility policy and State / Federal guidelines.
- Assists the facility in implementation of policies and procedures as it relates to restorative nursing care and reporting of resident condition, and level of assistance needed to ensure appropriate service delivery, offer accurate documentation on the MDS and State and Federal reimbursement requirements;
coordinates determining Assessment Reference Dates to ensure appropriate RUG scores.
- Assists with resident / patient care needs and comforts including but not limited to :
- Provides the direct rehabilitation / restorative care as delegated and taught by therapy and or nursing.
- RNP PROM, AROM, application of splints, braces, cones, prosthetic devices and care of limbs were applied, bed mobility, transfers, walking, dressing / grooming
- Assistive Feeding devices and restorative dining techniques; cueing and swallowing
- ncontinence management techniques
- Participates in restorative dinging programs : Walk to Dine
- Serve as a resource and role model for CNA’s
- Attends restorative meeting s and care plan conferences as directed
- Reports unusual problems regarding resident care to nursing staff immediately : i.e. pain, refusals, falls etc.
- Reports malfunctioning or improperly fitting equipment needs to therapy and restorative nurse
- Participates in the Quality Improvement Program as directed by the Restorative Nurse, and reports areas needing improvement
- Ensure the resident environment is as free from accidents and hazards as is possible. Immediately correct / address potential safety risks as prudent and warranted.
- Make on-going rounds on assigned unit.
- Evaluate needs of residents and report any problems to Charge Nurse.
- Ensure that all transfers and lifts are performed with concern for the safety of the resident according to the policies of the facility.
- Complete all documentation by the end of their shift :
- Point of Care
- ADL scoring
- Meal monitoring
- Bowel and Bladder
- Restorative programs
- Participate in patient care plan conferences.
REQUIRED SKILLS / ABILITIES :
- Able to relate positively, effectively, and appropriately with residents / patients, families, community members, volunteers, and other facility staff.
- Possess special interest in, and a positive attitude about, working with long-term care residents / patients and the elderly.
- Meets all health requirements, as required by law.
- Able to read, write, speak, and understand English.
- Demonstrates basic computer knowledge and ability with an aptitude to learn company applications.
- The responsibilities of this position involve significant physical activities including standing, lifting (up to 60 pounds unassisted), bending, stooping, pushing, pulling, and twisting.
Lifting and / or transferring some residents will require use of a lifting device and / or assistance of other staff.
EDUCATION and EXPERIENCE :
- Possesses current certification as a nursing assistant (CNA) required.
- Prior experience in a long-term care setting required.
ENVIRONMENTAL AND PHYSICAL REQUIREMENTS :
- Walking, reaching, bending, lifting, extended sitting, grasping, fine hand coordination, pushing and pulling, ability to distinguish smells and temperatures, all with or without the aid of mechanical devices is required.
- Limited potential for exposure to environmental hazards. Understanding and adherence to company safety standards and protocols required.
- Meet all health requirements as needed.
- Dialogue concerning ADA reasonable accommodations are always available upon request i.e. the interactive process.
