Prenatal Care Coordination (PNCC) is a Medicaid and BadgerCare Plus benefit that helps pregnant women get the support and services they need to have a healthy baby.
PNCC services include:
- Help getting the health care you need
- Personal support
- Information on good eating habits and health practices
- Help finding needed services in your community
The purpose of the Medicaid Prenatal Care Coordination program is to provide access to medical, social, educational, and other services to pregnant women who are considered high risk for adverse pregnancy outcomes. The components of this benefit are outreach, assessment, care plan development, ongoing care coordination and monitoring, and health education and nutrition counseling. Some key outcome indicators include: Tobacco Exposure, Alcohol Use, Breastfeeding, Safe Infant Sleep Practices, Perinatal Depression, Family Planning, and Involved Father.
Duties will include but not limited to:
·Provides nutrition classes for clients.
·Coordinates client services and referrals with appropriate agencies when necessary.
·Participates in case plan reviews and interdisciplinary team meetings.
·Develops and/or approves nutrition education materials.
·Completes annual Nutrition Education Plan.
KNOWLEDGE, SKILLS & REQUIREMENTS:
· Bachelor`s degree in public health nutrition, nutrition or related field
or
Certified Dietitian
·Knowledge of nutritional needs of pregnant, post-partum and breastfeeding women, infants, and children under the age of five years.
· 1-2 years of experience as a Dietitian/Nutritionist, preferably serving the underserved population.
· Ability to work with clients and staff with a variety of ethnic backgrounds.
· Effectively communicates with clients individually, as well as in group education sessions with a commitment to serving those in need.
·Provides excellent customer service to all internal and external customers.
·Experience with Microsoft Office required.
Job Types: Part-time, Contract
Pay: $25.00 - $35.00 per hour
