Summary: The Medical Transporter DSP will be responsible for scheduling the medical/dental appointments for the people we support in a timely fashion and provide transportation to the appropriate health care providers. As well as the responsibility for documentation and excellent communication. They are responsible for ensuring that services are provided within the standards established by the departments and the agency. They must understand, be committed to, and respect the people being served, their family members and the agency’s mission. They are responsible to ensure that the services provided are geared to promote personal growth, skill acquisition, and community inclusion. These services must be meaningful to the people served and encourage and respect their choices. Hours are variable based on the needs of the person receiving services. DSP responsibilities will be utilized as needed and are a requirement of the position. The main responsibilities are to ensure that all appointments are completed.
Note: The Medical Transporter assigned to the Church Avenue IRA will start work at 7am and assist with the passing of medications. When administering is completed, the MT/DSP will assist the individual’s morning routine as needed including with ADLs. The Medical Transporter/DSP will not be assigned anyone that has heightened supervision such as one to one. Once the individuals leave for the program the MT/DSP will transfer to the nursing department to begin transporting them to medical appointments.

All staff must pass all NYS Justice Center Background Check Requirements and Physical. In addition, the employee has a duty to report any suspected violations of the law (including any motor vehicle infractions) to the Human Resources Director.

Responsibilities
Med Transporter Responsibilities:
- Work with residential/nursing teams, schedule and attend all medical, dental, diagnostic testing and lab appointments as assigned or asked to fill in for other Med Transporters.
- Advocate for individuals at appointments.
- Responsible for tracking assigned appointments and ensuring that they are kept/and or rescheduled.
- Maintain Therap and house calendar for all medical appointments in caseload.
- Print and review all consults as directed by supervisor.
- Maintain medical calendar and review with Nursing department.
- Maintain ready to go folders (medications, insurance cards, immunization records, masks, etc.) for assigned location.
- Appointment information is scanned into Therap file, update nursing of outcome of appointment. Responsible for scheduling follow up appointments and ordering medications from appointment.
- Weekly inventory of medication carts in caseload, ensuring meds, supplies are ordered, in place. Clean and organize med cart/room; pay attention to supplies (cups, garbage bags, hand sanitizer, etc.)
- Responsible for assisting with AM/Afternoon medication passes as needed (at least once monthly).
- Attend mandatory meetings and training.
- Updating medical information on Therap.
- Scanning and filing as directed by supervisor.
- Reviewing the MAR binder and print scripts.
- Sending Consults one day in advance with behavior reports for psychiatry.
- Providing weekly and monthly follow up emails to nursing, residential, day programs and clinical.
- Proving coverage and monitoring after pre-sedates are given for appointments for individuals having procedures.
- Responsible for urgent care or ER visits when deemed necessary.
- Helping provide care when people need to be sent home sick.
- Updating diet plan cheat sheets and printing at program locations.
- Provide and maintain the highest levels of person-centered support to ensure effective, efficient delivery of services while serving as a model for support to all members of the team.
- Obtain and maintain In Flight, Inc. driver certification status to facilitate opportunities for meaningful community integration for the people supported by In Flight, Inc.
- Ensure proper documentation, including but not limited to programming services in Therap, GERs and behavior data in Therap and MAR documentation, is completed daily.
- Adhere to all agency standards, regulations and policies regarding time and attendance.
- Regularly review Behavior Support Plans and IPOPs or safeguards as required by the worksite for people being supported.
- Other directives given by supervisor.
- When the vehicles are grounded or there is a holiday falling Monday through Friday the Med transporter will be directed to support at the program locations as a DSP.
- Med transporters are able to pick up additional shifts and will report to the IRA Manager for any DSP related duties.
Requirements

- Pass all NYS and OPWDD background clearances.
- Must have NYS driver’s license that meets Insurance Agency requirements.
- Attend a week and a half orientation in Red Hook
- Must be med certified or become certified (to keep position)
