Overview
We are seeking a compassionate and dedicated Hospice Care Manager to join our team. The ideal candidate will play a critical role in providing high-quality end-of-life care to patients and their families. This position requires a strong background in healthcare management, with a focus on patient-centered care, effective communication, and interdisciplinary collaboration. The Hospice Care Manager will oversee the delivery of services, ensuring that all aspects of care are coordinated and tailored to meet the unique needs of each patient.

Responsibilities

  • Coordinate and manage hospice services for patients, ensuring comprehensive care plans are developed and implemented.
  • Collaborate with physicians, nurses, social workers, and other healthcare professionals to provide holistic support to patients and families.
  • Conduct assessments to evaluate patient needs and preferences, including pain management and behavioral health considerations.
  • Monitor patient progress and adjust care plans as necessary, utilizing evidence-based practices.
  • Provide education and support to patients’ families regarding hospice services, symptom management, and end-of-life issues.
  • Ensure compliance with regulatory requirements and maintain accurate documentation of patient care activities.
  • Participate in quality improvement initiatives to enhance service delivery and patient outcomes.

Pay: $83,213.00 - $86,973.00 per year
