Position Summary
The Advanced Practice Provider (APP) will provide high-quality, patient-centered care to individuals receiving care in a 14-bed Inpatient Rehabilitation Facility (IRF) at St. Josephs Hospital in Nashua, NH. The APP will work collaboratively with an interdisciplinary team, including physicians, therapists, nurses, and case managers, to optimize patient outcomes and support a smooth transition from acute care to rehabilitation.
This position ensures continuity of care, performs comprehensive assessments, and develops treatment plans aligned with the goals of rehabilitation and recovery.
Key Responsibilities
- Clinical Care
- Conduct comprehensive patient assessments, including history-taking, physical exams, and diagnostic evaluations.
- Develop, implement, and modify individualized treatment and rehabilitation plans.
- Monitor and manage acute and chronic medical conditions, adjusting care plans as needed.
- Prescribe medications, order and interpret diagnostic tests, and perform procedures within scope of practice.
- Medical consultations on the adult behavioral health unit to support comprehensive medical management.
- Collaboration and Teamwork
- Collaborate with the interdisciplinary team to address patient needs, establish care goals, and coordinate rehabilitation services.
- Communicate effectively with patients, families, and caregivers to ensure understanding of care plans and treatment goals.
- Patient Education
- Provide education and counseling on disease management, lifestyle modifications, and medication use.
- Support patients and families in navigating post-acute care plans and addressing barriers to recovery.
- Documentation and Compliance
- Document all patient interactions, treatment plans, and progress notes in the electronic health record (EHR) in a timely and accurate manner.
- Ensure compliance with facility policies, state and federal regulations, and professional practice standards.
- Quality Improvement and Professional Development
- Participate in quality improvement initiatives aimed at enhancing patient outcomes and care delivery.
- Engage in ongoing professional development to maintain expertise in rehabilitation medicine.
Qualifications
- Education:
- Masters degree in Nursing (NP) or Physician Assistant Studies (PA).
- Licensure/Certification:
- Current state licensure as a Nurse Practitioner for the state of NH (NP) or Physician Assistant (PA).
- National certification in area of practice (e.g., ANCC, AANP, NCCPA).
- Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) certifications.
- Experience:
- Minimum of 1-2 years of experience in inpatient care, rehabilitation, or a related field preferred.
- Skills:
- Strong clinical assessment and decision-making skills.
- Effective communication and teamwork abilities.
- Proficiency in using electronic health records (EHR).
Work Environment
- Setting: Inpatient Rehabilitation Facility, 14 beds.
- Schedule: Monday through Friday, 8AM-5PM. One weekend a month rounding. No night call responsibilities.
- 3 weeks vacation.
- 1 week CME.
- Base salary is $140,000.00 with earning potential of $160,000.00.
