The Inclusive Pathways Project LLC is currently looking for independent contractors in the following fields to join our growing team of transition consultants to support families both in Connecticut and across the United States in the transition planning process.
- Occupational Therapist
- Physical Therapist
- Licensed Clinical Social Worker (LCSW)
- Special Education Teacher
As a transition consultant with The Inclusive Pathways Project LLC, your role will be vital in supporting individuals with disabilities and their families as they navigate their journey toward a successful and fulfilling future. You will work as part of a multi-disciplinary team to facilitate the Pathway Planning process, focusing on helping individuals achieve their goals in employment, postsecondary education, independent living, and community participation.
Your primary responsibility will be to help gather and analyze essential data about the individual's past experiences, current strengths, interests, and future goals. By conducting one-on-one assessments and interviews, you will work closely with the individual and their family to ensure that all perspectives are accounted for and are thoroughly explored.
The Pathway Plan you help create will be a personalized roadmap that reflects the unique strengths, interests, and goals of each individual. You will work closely with a team of experts with diverse backgrounds and education to develop tailored ideas, resources, and strategies that align with the individual’s specific needs across all areas of transition, including employment, postsecondary education, independent living, and community participation.
Drawing on your personal/professional expertise, you will assist in identifying relevant opportunities—whether for education, training, housing, or community involvement—while also helping to identify and address potential barriers, such as skill gaps, accessibility issues, or support needs. You will collaborate with the individual and their family to offer creative, practical solutions, empowering them to take actionable steps toward achieving their future goals.
We are looking for a Transition Consultant who…
- Has experience/expertise in transition planning for individuals with disabilities
Can conduct and analyze various individualized assessment methods
Works collaboratively within a multidisciplinary team model
Has the knowledge to recommend relevant resources (training, education, assistive tech, etc.)
Can identify barriers and develop practical solutions
Can communicate effectively with individuals, families, and various other stakeholders
Helps set clear, achievable goals and action plans
Demonstrates cultural competence and adapts plans to meet diverse needs
Qualifications
Experience in working with individuals with disabilities and their families.
Experience with transition planning and/or individuals with disabilities ages 12 - 30.
Access to a reliable computer and internet connection.
Valid driver’s license.
Must hold the appropriate Connecticut state licensure or certification in your area of expertise (if applicable).
Job Type: Contract
Pay: $48.00 - $60.00 per hour
