Job Overview
We are seeking a highly skilled Registered Nurse to join our team as an Ambulatory Care Coordinator. This role requires a highly motivated and organized individual with excellent communication skills. As an Ambulatory Care Coordinator, you will be responsible for coordinating patient care, communicating effectively with multidisciplinary teams, and maintaining accurate records of patient progress and outcomes.
Key Responsibilities

Requirements
