Job Description:
We are seeking a highly skilled Transplant Coordinator to join our team at Incredible Health. This is a full-time position that requires strong communication and organizational skills, as well as a commitment to delivering high-quality patient care.
Responsibilities:
- Develop and maintain effective relationships with patients, families, and healthcare providers to ensure seamless communication and care coordination.
- Participate in multidisciplinary rounds and meetings to discuss patient progress and develop care plans.
- Ensure accurate and timely documentation of patient information, adhering to regulatory requirements and organizational standards.
Qualifications:
- ADN/ASN degree or higher from an accredited school of nursing.
- Active and unencumbered Registered Nurse license in the state of New Jersey.
- Minimum 2 years of experience in a related field, preferably in transplant services.
