About the Job:
We are actively hiring experienced Registered Nurses for our Home Health team in the Port Jefferson, NY area. Our ideal candidate will have a strong background in one or more of the following areas:
- Catheter care and placement
- Developing and implementing personalized care plans
- Providing patient and family education
- Skin assessment and ulcer prevention
- Wound and Stoma care
Responsibilities:
- Develop and implement comprehensive care plans tailored to individual patients' needs
- Provide skilled nursing care and support to patients in their homes
- Collaborate with interdisciplinary teams to ensure seamless care coordination
- Educate patients and families on disease management, medication adherence, and self-care techniques
- Conduct thorough skin assessments and provide interventions to prevent ulcers and promote wound healing
- Administer medications, perform treatments, and conduct vital sign checks as needed
Qualifications:
- RN Diploma degree or higher from an accredited school of nursing
- Active and unencumbered Registered Nurse license in the state of New York
