Job Summary:
The RENAL DIETITIAN participates in the care of the ESRD patient, development of nutritional care plans, documentation of nutritional history, assessment, plan instruction, and progress towards goals, as well as all other relevant duties and those assigned by the Director of Clinical Services or the Lead Dietitian.

Principle Duties and Responsibilities:
- Documentation of nutrition assessment, plan of care, instruction, and development of goals as defined under Conditions of Coverage.
- Participates as an interdisciplinary team member in developing long and short-term care plans as defined under Conditions of Coverage.
- Intervention with hospitals, nursing homes and personal care homes regarding patient?s nutritional health.
- Patient, family and staff education
- Provides information and referral (Meals-on-Wheels, nutritional supplements)
- Assists in the development of the dialysis prescription based on Kinetic modeling and established protocols
- Provides assistance with CQI program where applicable
- Maintains the confidentiality of all Practice information regarding patients, coworkers and physicians
- Performs all other relevant duties which may be contribute to the patient?s well being as they relate to nutrition and diet, as well as those assigned by the Director of Clinical Services
Required Knowledge, Skills and Abilities:
- Must possess ability to exercise expert judgment and develop insight into delivering appropriate care
- Must possess ability to communicate effectively and express ideas clearly and concisely as this position requires frequent contact with patients, physicians, and nursing staff
- Must be skilled in the use of Excel, Word and other computer based programs
- Must possess ability to meet physical demands of the position. These include, but are not limited to, constant sitting, standing, bending, lifting and moving. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions
- Ability to excel in stressful, fast-paced work environment subject to frequent interruptions and hostile and emotionally upset patients, family members, staff, etc. Continuously serves as a mediator between patients , family members and Practice staff
