What are the responsibilities and job description for the Determination Coach/Case Worker position at Independent Facilitation Services?
We are seeking a motivated and compassionate Determination Coach/Case Worker to support a young lady on the spectrum to achieve her personal goals of greater independence and quality of life. The ideal candidate will have experience in case management, particularly one-on-one support for ASD clients, managing employees, and advocacy. Proficiency in individualized planning for people with disabilities is essential.
Key Responsibilities
- Goal Setting
- Collaborate with participants and their families to identify and set achievable short-term and long-term goals based on individual strengths, interests, and needs.
- Individualized Planning
- Develop and implement personalized support plans outlining steps to reach participants' goals, including accessing services, skill development, and community integration.
- Advocacy
- Advocate for participants to ensure they receive appropriate services, accommodations, and resources from the community and various providers.
- Case Management
- Provide ongoing case management by managing employees and coordinating with healthcare providers, educators, social services, and other professionals for comprehensive support.
- Skill Development
- Assist participants in developing life skills such as communication, social interaction, self-care, and problem-solving to increase independence.
- Resource Coordination
- Connect participants and families with community resources, including educational programs, vocational training, therapy services, and recreational activities.
- Progress Monitoring
- Regularly assess and document participants' progress toward their goals, adjusting support plans as needed for continued growth.
- Family Support
- Provide guidance, support, and education to families, helping them understand their loved one’s needs and contribute to their development.
- Crisis Intervention
- Offer immediate support during crises to ensure participants' safety and coordinate with other professionals as necessary.
- Documentation
- Maintain detailed records of participant interactions, progress, and changes to support plans, ensuring compliance with regulations.
- Proficiency in Microsoft Office products, including Office 365.
- Policies for Time Off
- Submit written vacation requests at least two weeks in advance for approval.
- Notify the supervisor at least two hours before the start of the shift if unable to work due to illness.
- Other Duties
- Perform additional responsibilities as assigned.
Qualifications
- Experience:
- Previous experience in case management, social work, or coaching individuals with disabilities is required.
- Education:
- Bachelor’s degree in Social Work, Psychology, Special Education, or a related field is preferred. Certification in case management or coaching is a plus. Experience may substitute for education.
- Skills:
- Strong communication, empathy, problem-solving, and organizational skills.
- Ability to work effectively with individuals from diverse backgrounds and abilities.
- Certifications:
- Current CPR and First Aid certification preferred.
- Transportation:
- Valid driver’s license and reliable transportation for home visits and community outings. Mileage reimbursement is provided.
- Background Check:
- Must pass a background check and provide professional references.
