Description
The Caregiver Support Coordinator is responsible for providing guidance, resources, and advocacy to caregivers of individuals with disabilities, chronic illnesses, or aging-related needs. This role ensures caregivers receive the support, education, and respite necessary to maintain their well-being while effectively caring for their loved ones.
Independent Living, Inc., is a progressive and dynamic cross-disability services organization and a leader in the provision of peer services in the Hudson Valley region. The Caregiver Support Coordinator position requires bachelor’s degree in social work, human services, healthcare, or a related field, or equivalent experience (master’s degree preferred).

Duties & Responsibilities:
- Provide services to adults aged 60 or older that have a chronic illness or disability as well as their caregivers related to:
- Information and Assistance.
- Counseling, Support Groups and Training.
- Respite Care which includes, but is not limited to, in-home care aides, in-home contact and support, and adult day services.
- Coordination of Supplemental Services which include, but is not limited to, transportation, home-delivered and congregate meals, nutrition counseling and personal emergency response.
- Assist caregivers with gaining knowledge and/or skills related to their caregiving role.
- Provide consistent, accurate, and cost-effective information that is available to all eligible consumers.
- Assist consumers with connecting to services and resources that are available within their community.
- Assist the caregiver with supplemental services that include, but are not limited to, personal emergency response systems, assistive technology, home modifications, disposable supplies nutrition services and transportation.
- Complete reports for the Program Manager by a specified due date.
- Complete all reporting and record keeping as required.
- Attend and actively participate in all meetings and training sessions provided by Independent Living, Inc. and specific to the job responsibilities of the position.

SHIFT:
- Monday through Friday 9am-5pm.
PAY RATE:
- $23.45/hr - $25.00/hr

Requirements
- Bachelor’s degree in social work, human services, healthcare, or a related field, or equivalent experience (master’s degree preferred).
- Experience in caregiver support, disability services, aging services, or behavioral health.
- Strong understanding of community resources, benefits, and best practices in caregiving.
- Excellent communication, problem-solving, and organizational skills.
- Demonstrated ability to recognize the need for and facilitate connections between consumers and Independent Living, Inc. (ILI) and other disability related services.
- Ability to work independently and collaboratively with diverse populations.
- Must possess a valid, unrestricted driver’s license and have reliable transportation.
- Must have the ability to work a flexible schedule, including weekends if applicable, based on the needs of the program or agency.
- Proficiency in Microsoft Office and database management.
- Bilingual preferred.
