Independent Living Partners is seeking experienced and driven ILS/IHS workers to join our growing team. ILPS is a person-centered social services agency. Individualized Home Supports (IHS), also known as ILS, is an active role that involves bending, walking, and hands-on work. In this position, you will not only assist clients with various activities but also participate alongside them to ensure tasks are completed efficiently. In many cases, you will independently complete activities that help individuals maintain a dignified and independent life.
Primary Responsibilities
- Complete laundry and general housekeeping tasks
- Assist individuals in organizing their lives and working toward a fulfilling life
- Assist individuals set up meetings and appointments
- Arrange medical and social services
- Complete or provide assistance with task completion
- Support individuals with problem-solving
- Assist with meal planning
- Assist with cooking and meal preparation
- Arrange or provide transportation for individuals
- Support individuals in safely accessing the community for socialization with friends, family, and peers
- Assist individuals in safely accessing the community for recreational activities
- Assist individuals improve communication and social skills
- Support individuals with self-care
- Assist with paperwork and organization
- Assis individuals shop for groceries and other essentials
- Create a work schedule that aligns with the client’s needs
- Complete daily case notes and time sheets
- Provide assistance with Activities of Daily Living (ADLs) to maintain client dignity
- Remind and assist clients dress appropriately for the weather and social outings while ensuring their dignity is safeguarded
Training & Qualifications
- Meet DHS qualifications for Individualized Home Supports with training (formerly known as ILS)
- Experience working with individuals with disabilities
- Patience and understanding in working with individuals experiencing mental health and behavioral challenges
- Associate’s degree preferred
- First Aid certification required
General Skills & Requirements
- Ability to work with individuals with disabilities and chronic illnesses
- Experience supporting individuals with mental health conditions
- Assist clients in identifying and implementing skills to promote independence
- Complete 245D HCBS training upon hire and annually
- Ability to pass a Minnesota Department of Human Services Background Study
- Valid driver’s license with fewer than three moving violations in the last three years (including DUIs)
- Maintain a reliable vehicle with valid insurance
- Access to a computer with internet and email (PCA provided if needed)
- Maintain a cell phone for communication
- Strong time management skills
- Effective communication skills, with the ability to empathize and connect with individuals
- Maintain responsive communication with the agency and supervisors
