Purpose Statement :
The role of the Direct Support Professional is to support individuals in their home. Identify and support each Vulnerable Adult achieve goals and improve their lives by creating opportunities for independence. The Direct Support Professional is responsible for providing personal care, and designated health-related service designed to maintain the physical and emotional well-being of the vulnerable receiving services from Independent Living Services of Central Minnesota.
Job Roles / Responsibilities :
- Understand standard precautions used for infection control and adhere to them
- Assist person served with activities of daily living as indicated on their individual service agreement
- Assist the person served with daily room order and light housekeeping
- Assist the person served with personal laundry duties, which may include ironing
- Maintain a clean, safe, and healthy environment
- Promote independence, not dependence with the person served offering choices and fostering self-help skills
- Document any changes in the person served condition in the person served Notes in documentation platform
- Report these changes to the Designated Coordinator in a timely manner
- Completes Incident Reports according to policy
- Reporting directly to the person served and their external team on the weekends when needed
- Understands procedures regarding home care tasks and charting procedures when performing these tasks
- Recognizing abnormal signs and symptoms, or changes in person served condition
- Documenting duties completed with person served on the service delivery record during each shift
- Filing papers in person served charts as needed
- Completing Notes in documentation platform for every shift worked
- Clocking into and out of documentation platform and having the location on
- Documenting Notes and Goals in documentation platform for every shift worked
- Reviewing and signing all new person served plan of care / weekly service record and changes in current person served plan of care
- Checking outgoing US mailbox, collecting US mail delivered and distributing US mail and in-house mail to the person served
- Other documentation as assigned and / or needed
- Understand and adhere to the Vulnerable Adult and abuse policy, Bill of Rights and HIPPA
- Works to assure facility is in compliance with federal, state, and local standards and regulations
- Complete mandatory online annual trainings modules -assigned and due monthly
- Staying up to date on all Person Specific Training
- Encourage, gather and / or transport person served to participate in scheduled activity programs
- Lead activity programs as assigned
- Socialize with person served and offer new opportunities to incorporate into their lives
- Develop and maintain a positive working relationship with ILSCM staff
- Provide backup to other staff as needed
- Communicate information regarding person served and the care needs and changes to the team and families when appropriate
- Provide proactive and constructive participation in staff meetings
- Be compassionate and work with tact and ethical awareness
- Be flexible and adaptable to changing situations
- Responds appropriately to safety hazards, fire, weather, or emergency situations
- Follow through promptly on requested duties
- Meal preparation at mealtimes, set up, serving and clean up
- Provide on-the-job training for new associates
- Must be mature and emotionally stable using a calm and kind tone of voice
- The ability to remain calm in difficult or unusual circumstances
