We are looking for an SMI (Serious Mental Illness) Care Manager. Independent Living Systems (ILS), together with its affiliated health plans, Florida Community Care and Florida Complete Care, are committed to improving the quality of life for the most vulnerable populations. You will also conduct health risk assessments to identify needs and gaps in care, collaborating with members to develop personalized care plans that address these needs and support them in achieving their health goals.

Additionally, your capacity to work both independently and collaboratively as part of a team will be key to delivering high-quality care to our members.

Minimum Qualifications
- Bachelor's degree in social work, sociology, psychology or a related field
- Registered Nurse (RN) or Licensed Practical Nurse (LPN) licensed to practice in the state of Florida
- Minimum of 2 years of experience working with individuals with mental illnesses, in a case manager or similar role
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Bilingual in English and Spanish is preferred
- Relevant experience may substitute for the educational requirement on a year-for-year basis
- Master's degree in social work, psychology, nursing, or a related field
- Experience in care management or case management
- Knowledge of community resources and services
- Bilingual in English and Spanish
- Certification in mental health or related field
- Develop and implement care plans for individuals with serious mental illness
- Collaborate with healthcare professionals to ensure that patients receive comprehensive care
- Monitor patient progress and adjust care plans as needed
- Provide education and support to patients and their families
- Maintain accurate and up-to-date patient records
