Responsibilities:
- Empower residents to be as independent as possible.
- Facilitate wellness by promoting activities and a healthy lifestyle.
- Develop and arrange educational/preventative health programs and services for residents.
- Educate residents, families, and staff on available community resources.
- Assist residents in building informal support networks among themselves.
- Acts as a liaison between community agencies, service providers, and residents.
- Ensure resident services are rendered appropriately, timely, and satisfactorily.
- Conduct comprehensive, non-clinical assessments of residents for wellness and social need.
- Observe resident confidentiality while assisting with community service applications.
Job Qualifications:
- B.A in Social Work, Gerontology, Psychology, Public Health or related field preferred.
- Experience working with Senior Population in Social Service setting.
- Superior interpersonal and organizational skills.
- Demonstrated working knowledge of community services in the region.
- Experience in service management, organization, problem solving, and advocacy.
- Experience in elder services, disability services, substance abuse, and mental health services.
- Possess crisis intervention skills.
- Adherence to Federal Fair Housing Laws.
