Job Summary
This role supports two important programs under our Integrated Behavioral Health Team-Memory Care and Domestic Violence Prevention-and combines clinic-based care with opportunities for community outreach and education. The LICSW will work closely with the medical team, integrated behavioral health team, program staff, and community partners. Essential Functions
Domestic Violence Prevention Program
Provide direct support to individuals affected by violence, including safety planning, resource referrals, and collaborating with others to address patient's various intersectional needs
Memory Care Program
Provide supportive counseling to address immediate needs, build coping skills, and foster healing for caregivers and those with dementia

Physical Demands
The work requires some physical exertion such as long periods of standing, walking, and recurring bending, crouching, stooping, stretching, reaching, or similar activities. The work may require specific, but common, physical characteristics and abilities such as above average agility and dexterity. An individual in this position may be exposed to adverse indoor and outdoor environmental conditions, blood borne pathogens, physical hazards (e.g. needle pricks, etc.) and chemical hazards
