    Job Summary

    This role supports two important programs under our Integrated Behavioral Health Team-Memory Care and Domestic Violence Prevention-and combines clinic-based care with opportunities for community outreach and education. The LICSW will work closely with the medical team, integrated behavioral health team, program staff, and community partners.     Essential Functions

    Domestic Violence Prevention Program

  • Collaborate with the medical team to provide services and meet the needs of patients who have disclosed a history of violence
  • Provide direct support to individuals affected by violence, including safety planning, resource referrals, and collaborating with others to address patient's various intersectional needs

  • Offer brief supportive and psychoeducational counseling to individuals affected by interpersonal violence, focusing on fostering resilience, promoting healing, and addressing immediate needs
  • Build partnerships with local advocacy groups, shelters, and support services to create a robust referral network for comprehensive patient care
  • Contribute to the design and implementation of patient-centered interventions, such as self-esteem groups, support groups, or psychoeducational book clubs. The LICSW is encouraged to bring forth ideas and take initiative to develop innovative offerings that align with organizational goals
  • Memory Care Program

  • Collaborate with the medical team to provide services and meet the needs of those who are affected by dementia
  • Provide supportive counseling to address immediate needs, build coping skills, and foster healing for caregivers and those with dementia

  • Promote community awareness and education about cognitive health to help reduce stigma of seeking medical care for these concerns
  • Create opportunities for the detection of cognitive impairment in clinic and at community events
  • Oversee an interdisciplinary dementia care team to conduct assessments and develop care plans
  • Develop a comprehensive resource guide of local dementia resources, and tribal resources, and develop relationships with community organizations
  • Support individuals with dementia and their caregivers by designing and implementing person-centered interventions, such as support groups, educational workshops, and tailored activities. The LICSW is encouraged to propose innovative ideas that align with organizational goals to enhance caregiver well-being and promote resilience
  • Other duties may include providing brief assessment and intervention for patients presenting to primary care for other behavioral health conditions
  • Knowledge, Skills, and Abilities

  • Knowledge of trauma-informed care principles and local community resources
  • Knowledge of American Indian history and cultural traditions
  • Detail oriented and self-motivated
  • Strong relationship skills and ability to work within a team
  • Excellent communication skills
  • Ability to show initiative and solve problems, manage one or more projects, be accurate and attentive to deadlines while working independently
  • Willingness to learn
  • Bilingual skills are a plus
  • Education and Experience

  • Master's degree in Social Work (MSW) from an accredited institution
  • Licensed in the State of MN
  • Preference for candidates with experience in working with elders and / or a background in working in domestic or sexual violence advocacy.
  • Experience and understanding of Native American / Indigenous culture and community
  • Physical Demands

    The work requires some physical exertion such as long periods of standing, walking, and recurring bending, crouching, stooping, stretching, reaching, or similar activities. The work may require specific, but common, physical characteristics and abilities such as above average agility and dexterity. An individual in this position may be exposed to adverse indoor and outdoor environmental conditions, blood borne pathogens, physical hazards (e.g. needle pricks, etc.) and chemical hazards
