The work requires some physical exertion such as long periods of standing, walking, recurring bending, crouching, and stooping, stretching, reaching, or similar activities; recurring lifting of children, and lifting in excess of 50 pounds. The work may require specific, but common, physical characteristics and abilities such as above-average agility and dexterity. The work environment involves high risks, with exposure to potentially dangerous situations requiring a range of safety and other precautions.

Job Summary
The Nursing Supervisor is responsible for managing day-to-day patient flow in the medical clinic. This position will be responsible for coordinating activities related to clinical quality improvement, medical home, and care coordination activities.
Essential Functions
- Supervise clinical nursing staff (LPNs and MAs) and other staff as assigned
- Manage day to day patient flow within the clinic
- Assign care coordination activities to clinical nursing staff including pre-visit planning and the development of care plans.
- Performs general ambulatory health care to patients, including administration and/or dispensing of prescribed medication and treatments in accordance with clinic policies and procedures.
- Ensure compliance with nursing quality activities
- Provide patient triage in person and on phone
- Support providers in care coordination of select patients and/or select patient groups
- Perform clinical assessments of patients
- Perform patient education, self- management support, pre-visit planning and care plan development
- Coordinate expansion of PCMH throughout the clinic.
- Manage medical equipment to maintain availability to providers as well as cost effectiveness and compliance with Joint Commission.
- Ensure effective communication between patient entry to care and care team (i.e. effective communication and teamwork between front desk/nursing/providers)
- Identify and report clinical, utilization and outcomes issues. Identify workflow issues and recommendations for improvement.
- Lead projects to improve outcomes and quality measures.
- Maintains and implements department and organizational policies and procedures
- Maintains nursing standards, policies and procedures manuals and workflows.
- Manage the medication management standards within IHB to maintain compliance with Joint Commission and MNVFC including sample medications, dispensed medications, onsite administered medications.
- Provide health promotion and disease prevention education to IHB Patients.
- Back-up nursing staff when needed and will work an occasional Tuesday night.

- Licensure & Education:
- Current Registered Nurse (R.N.) license and registration to practice in the State of Minnesota.
- Bachelors degree in nursing; minimum 3-5 years of clinical experience in a healthcare setting.
- Clinical & Operational Experience:
- Experience in clinical operations, including workflow management and patient care coordination.
- Experience leading quality improvement initiatives to enhance patient care and clinical efficiency.
- Knowledge of the Medical Home Model of care preferred.
- Leadership & Supervisory Experience:
- Supervisory experience required, including mentoring, training, and evaluating nursing staff.
- Experience in care coordination services, particularly in community health or underserved populations.
- Professional & Interpersonal Skills:
- Strong relationship-building skills with patients, medical staff, and external partners.
- Ability to interpret, adapt, and apply guidelines and procedures effectively.
- Excellent communication skills, both written and verbal.
- Self-motivated with strong critical thinking and problem-solving skills.
- Compliance & Systems Knowledge:
- Familiarity with OSHA, HIPAA, and infection control protocols.
- Experience working with electronic health records (EHR) systems, EPIC preferred.
- Ability to understand and meet the needs of the patient through respectful, courteous and caring interactions with patients, families and other health professionals.
- Knows, understands and adheres to organizational policy related to the patient's rights for confidential care.
- Knowledge and respect of the American Indian culture and other cultures served by IHB.
- Actively participates and works positively, flexibly and cooperatively in a team effort to accomplish the goals of the Indian Health Board of Minneapolis.
