What are the responsibilities and job description for the PATIENT CARE COORDINATOR position at INDIAN HEALTH CARE RESOURCE CENTER?
The Patient Care Coordinator (RN), is responsible for assessing patients and determining their level of need for medical assistance. The criteria used to evaluate a patient include the type of injury or illness, its severity, symptoms, patient explanation of emergency, and vital signs.
ESSENTIAL JOB FUNCTIONS
- Provide safe, compassionate, and focused nursing care to each individual patient.
- Conduct assessment to meet patients’ need for medical, social, and mental assistance.
- Schedule appointments as needed.
- Ensure accurate and timely documentation of calls and to return calls in a timely manner.
- All other responsibilities as directed by providers/nurse management and all other duties.
- Back up nurse for same day/walk in clinic.
- Process patient prescription refill requests.
- Address chart notifications per medical providers.
- Thoroughness and accuracy in triaging patients and prioritizing based on acuity and provider schedule, using critical thinking skills.
- Adherence to triage guidelines for adult and pediatric patients.
- Competency in taking and recording vital signs.
- Ability to dispense medication to pediatric patients per triage guidelines.
- High School diploma or equivalent.
- Completion of an accredited RN program. Current and valid Oklahoma RN license.
- 2 years of clinical experience preferred.
- BLS CPR Certification.
