Under the general supervision of the CMO, the Physician will work within the limitations of their license in providing direct care services to the patients of FNCH. The Physician shall provide safe, direct patient care utilizing the medical processes in the ambulatory/primary care setting. The Physician shall provide clinical leadership for Primary Care staff and assist with other Primary Care duties as needed. The Physician shall serve as a positive role model to promote a positive work environment and provide quality patient care. Ensures adherence to FNCH/departmental policies and procedures and fully complies with all HIPAA mandates. The Physician will work closely with Behavioral Health Services and the Homeless Outreach Program, appropriate staff and administration to identify, develop and implement the comprehensive clinical services and preventative services to improve the health of FNCH clients and assist in reducing the barriers to care. Duties: 1. Assesses the patient's condition and medical needs, sets goals and utilizes appropriate clinical actions to achieve goals; 2. Provide direct health care services in an effective and professional manner. The services provided include but are not limited to: prevention/education, screening, urgent care, primary care, evaluations, counseling/therapeutic treatment, crisis response and other services which may be required to offer comprehensive medical services; 3. Provides health services to patients of all age groups according to recognized standards of care. 4. Ensures that direct patient services are provided in an effective, culturally appropriate manner; 5. Identifies and reports on problems/issues relative to access, barriers to services, and unmet needs; 6. Develops infrastructure and innovative services/projects in response to managed care; 7. Ensures accurate and thorough documentation of all patient visits and encounters; 8. Ensures support staff follow physician orders; 9. Ensures that medical equipment is maintained, monitors medical supplies and orders when necessary. 10. Assists with maintaining stock levels; 11. Records and reports pertinent observations and reactions regarding patients to medical staff; 12. Administers medications and performs routine and complex procedures and treatments; 13. Provides urgent care/emergency services; 14. Establishes guidelines for triage, trains staff in triage, and assists in handling triage calls; 15. Ensures that patient medical records contain the necessary information( i.e., documents medical history of chief complaint, assists in triage and initiates patient care for walk-ins.) 16. Work collaboratively with in-house and community providers for referrals and services; 17. Documents labs, cultures, ensures follow-up on abnormal results; 18. Follows patient safety related policies and procedures; 19. Demonstrates a proactive approach to patient safety by seeking opportunities to improve patient safety. 20. Identify and report conditions or situations which may put a patient at undue risk; 21. Report potential or actual patient safety concerns, medical errors and/or near misses in a timely manner; 22. Encourage patients to actively participate in their own care by asking questions and reporting treatment or situations that he/she does not understand; 33. Perform other duties as requested by CMO Qualifications: 1. Licensed and certified as a physician in the State of New Mexico 2.
