The Chronic Disease Case Manager will promote health care delivery across the continuum that will enhance the client’s quality of life, decrease fragmentation of care, while efficiently utilizing resources and containing cost. They will provide resource support to the Case Management Team. The Chronic Disease Case Manager will use a process that includes screening and case finding, comprehensive multidimensional assessment, care/discharge planning, connection with available resources, implementation of the plan, and on going monitoring and re-assessment. The plan sets goals and time frames for achieved goals that are appropriate to the individual and his/her family and are agreed upon by the family and the treatment team. This position reports directly to the Outpatient Services Program Manager. Duties: Establishes and maintains standards of patient care by providing leadership and direction for Inpatient and Emergency Room Staff in all areas of nursing practice. Responsible for compliance with Joint Commission accreditation, state and federal guidelines, Medicare and Medicaid criteria, conditions of participation, Managed Care and IHS guidelines, and nursing statutes in relation to the Alaska Board of Nursing. Provides supervision for the programs, assuring staffing is appropriate to meet program, patient and school of nursing preceptor needs, training is given as needed and approving work schedules for all unit staff. Coordinates services with other programs. Serves on various hospital committees as assigned. Encourages and facilitates participation by nursing staff in Unit and MHC activities. Participates in organizational activities and provides leadership in program improvement activities. Maintains Program Budgets with budgetary guidelines makes revisions to budgets as necessary, and promptly reports any discrepancies to the DON. Coordinates collection, analysis, and preparation of information required to evaluate nursing services provided, and reports in a timely manner. Assists with research and development of Nursing Policies, Procedures and Protocols.Implements standardization of Nursing Policies and Procedures. Ensures consistency with association and MHC wide policies and procedures. Participates in activities, which promote professional growth and fulfillment of goals for self and staff. Identify and monitor staff development needs and make referrals as indicated to nursing education/staff development. Maintain nursing staff by recruiting, selecting, orienting and training clinical staff with prudent utilization of Agency nurses. Ensures that nurses are current in competency, assessments, licensure, certifications and other annual training. Manages all Inpatient and Emergency Room nursing staff disciplinary actions and implements Performance Improvement Plans as needed. Completes Performance Reviews on all staff in a timely and appropriate manner. Demonstrates and holds all staff members accountable for a positive customer service attitude at all times. Utilizes team building, problem solving skills, and lean principles, in the on-going quality improvement initiatives. Qualifications: Current licensure as a Registered Nurse in the U.S. with at least two years Med/Surg experience and additional Case Management experience preferred. Must be able to pass the core competencies assigned to this position and maintain the educational requirements of the department. Ability to utilize various computer programs, including EHR/RPMS, Microsoft Office, and Excel database programs, iCare, CMET, AFHCAN, Vidyo, and Cerner. Demonstrates a wide theory base and sound clinical skills in order to interact in an effective manner with physicians, health team members, community agencies, and clients/families with diverse opinions, values, and religious and cultural ideals. Ability to work autonomously with little direction and be directly accountable for practice. Demonstrates leadership qualities including time management, verbal and written communication skills, problem solving, decision making, priority setting, work delegation, and work organization. Ability to lift up to 70 pounds and spend long periods of time standing or walking. Employee may be exposed to communicable diseases and hazardous materials.
